Describe the bug

Site affected: annotate.gate.ac.uk
Reported by Mughda:
An annotator could not add anymore annotation because they have more than one annotation in a document. Even when removed from the project and added to a new one, this did not fix the problem. Email of the user is shared in the Skype chat.

Annotator had to create another account to do annotation.

Definitely "more than one annotation in a document"? If it's the error message about more than one pending annotation then it could be another instance of #374?
