[CAL-4373] Dynamic host assignment with Routing Forms

Currently, we can route to specific event types based on rules. However, this requires creating a new event type for every rule, even when the only change needed is the assigned hosts. We want to simplify this by introducing routing forms that use a single integrated event type (likely hidden from regular event types) and dynamically assign hosts based on input and team member attributes.

Feature details:

• Add attributes to team members (already possible)
• Routing:
  • Instead of redirecting to different event types based on specific rules, we want to make rules that dynamically assign hosts to the same event-type based on user input and host attributes.
  • Example rules: if company size < 10 and the country = US, assign all hosts that have the following attributes: region = US, department = customer support
  • We want to allow multiple rules like that + a fallback rule
• After form is submitted, they will be redirected to the event type with the hosts assigned according to their input data.
• Event type settings: We may also want to allow users to configure more detailed event-type settings, such as the event type name and other advanced options.
