Could not save the form after clicking on Save button.

Description:
After filling all the form while adding new partner, the form could not be saved though the all valid details are entered in the form.
The error message is generated as "Error while submitting the form".

Steps to reproduce:

1. Firstly, enter the "DVS" url as "https://dvs-dev.naxa.com.np/".
2. Login to the system as super admin.
3. Click on the "Data Bank".
4. You will be navigated to admin dashboard.
5. And then click on the "Partners" sections.
6. Try to add new partner by clicking on "Add New" button.
7. You will navigated to two forms, fill up the forms by entering valid data and try to save it.
8. Observe the result.

Expected Result:
After entering all the valid data user should able to add new partner.
