Support creating multiple instances of the on-prem provider

In issue 7474 we allowed for multiple instances of the same cloud provider and the on-prem provider should have the same ability

We need to allow for multiple instances of on-prem providers. If different business units within on company are making use of a single platform it may be required for them to maintain separate credentials and admin users. In addition if the on-prem provider is used to front multiple different platforms, such as one being physical hardware while another is VMs the control and placement of these entities may need to exists in distinctly separate pools.
