Bump to rebar3, open up Repo

Rebar3 is used for the book content on open source Erlang.

The repo structure has otherwise been split up, attempts at non-PDF formats been reverted (they failed), and contributing guidelines have been established.

Minor fixes

• Fixing pagination
• Fixing running headers
• Reworking ToC to omit overlong exercise listing
• More precise wording around some memory issues
