The words "Table of Contents" appear on page 3 by themselves. Any tip on how to prevent this? Thanks to Lynne for reporting.

Fixed in 06c024e:

Changed the CSS to not page break before the ToC, support keepWithPrevious, and deal better with how some artwork, figures and tables break across pages. Fixes issue #511.
