[Uptime UI] Amsterdam related UI issues

This is a collection of UI issues I have found. Most of them are related to the new Amsterdam theme. I don't see them as blockers.

Enable status alert switch

As users can not enable status alerts as long as they haven't set the default action, the switch should be disabled in this case.

Monitor type should be sentence case

Please update the monitor type to start with an upper case letter and be consistent with the monitor list.

Thanks Katrin! WRT the map, Shahzad, Katrin, Paul, and myself favor of its removal but we never got a response from Drew. The tracking issue is #86485 . I've added it to our next refinement so we can finally put this to bed . We discussed this in slack as well. Slack thread: https://elastic.slack.com/archives/CE4MRBU48/p1616088261024400 Paul, do you think we need Drew's approval here, or can we proceed with the map's removal? It seems to always need attention every other release one way or the other, and is a heavy dep.

@katrin-freihofner . This switch is currently a trigger for a popover. Disabling the switch will cause the popover to not function appropriately.

@paulb-elastic This isn't currently labeled as a bug, but seems like a UI bug and something we can merge into 7.13?

@dominiqueclarke if capacity for 7.13, then I am fine with that (we were planning on doing them in 7.14, but happy to tackle now). There's also #96122 in the same camp.

@liciavale FYI, the additional length of Katrin's suggestions for the monitor status alerts toggle made it a bit crowded when placed to the right of the title, so I opted to place it underneath. Let me know if that's alright with you.
