usbphy0 support in the sunxi usb-phy driver has been merged, but the
dtsi's for sun4i/sun5i haven't been updated. This results in the phy
driver failing to load, breaking usb support.

The Ippo q8h has its serial console connected to the r-uart. Adjust the
serial0 alias to match.

This fixes the kernel serial console no longer working since 3.19-rc1, because
8250_dw.c now honors dt aliases, causing the serial console to be ttyS5 rather
then being ttyS0, as it was in 3.18 and before.

Note that adjusting bootargs instead is not an acceptable fix, because
console=ttyS0,115200 is used by a lot of bootscripts, etc. and this should
continue to work.

This patch fixes the following problem: data transmission in direction
IN break unless the GSNPSID register access is done with spinlock held.
This issue occurs at least in Exynos4412 SoC, probably in many SoC's
from Exynos familly.

In practice failure to find phy when requested in non-OF
case means it will never become available, so
__usb_find_phy() must return -ENODEV and not -EPROBE_DEFER.

This fixes a regression caused by commit 9c9d824
(usb: phy: Fix deferred probing), where the USB
controller driver is left infinitely into deferred probe
when there are no phys.

Testing has shown that on sun4i the display backend engine does not have
deep enough fifo-s causing flickering / tearing in full-hd mode due to
fifo underruns. This can be avoided by letting the display frontend engine
do the dma from memory, and then letting it feed the data directly into
the backend unmodified, as the frontend does have deep enough fifo-s.

Note since u-boot-v2015.01 has been released using the de_be0-lcd0-hdmi
pipeline on sun4i, we need to keep that one around too (unfortunately).

Looks like the failure is caused by arm64 uapi/asm/sigcontext.h, which
includes linux/types.h while other archs not.

Current perl consider this problem:

However there are users which use old version of perl.

This patch includes stdbool.h before Context.xs and define HAS_BOOL to
prevent perl'e headers define its own 'bool'. Code is learn from perl's
git tree.

Don't use the ins's ->sncprintf() if the parsing failed.

For example, this fixes the display of "imul %edx".  Without this patch:

When a dso contains multiple symbols which have same name, current
dso__find_symbol_by_name() only finds an one of them and there's no way
to get the all symbols without going through the rbtree.

So make symbols__find_by_name() return the first entry with the given
name and the next patch in this series will provide a way to iterate
from there, by the name ordered rb_tree, till a suitable symbol is
found.

The find_probe_trace_events_from_map() searches matching symbol from a
map (so from a backing dso).  For uprobes, it'll create a new map (and
dso) and loads it using a filter.  It's a little bit inefficient in that
it'll read out the symbol table everytime but works well anyway.

For kprobes however, it'll reuse existing kernel map which might be
loaded before.  In this case map__load() just returns with no result.
It makes kprobes always failed to find symbol even if it exists in the
map (dso).

To fix it, use map__find_symbol_by_name() instead.  It'll load a map
with full symbols and sorts them by name.  It needs to search sibing
nodes since there can be multiple (local) symbols with same name.

The commit dfef99c ("perf probe: Use ref_reloc_sym based address
instead of the symbol name") converts kprobes to use ref_reloc_sym (i.e.
_stext) and offset instead of using symbol's name directly.  So on my
system, adding do_fork ends up with like below:

As you can see, the do_fork was translated to _stext+456136.  This was
because to support (local) symbols that have same name.  But the problem
is that kretprobe requires to be inserted at function start point so it
simply checks whether it's called with offset 0.  And if not, it'll
return with -EINVAL.  You can see it with dmesg.

  $ dmesg | tail -1
    [125621.764103] Return probe must be used without offset.

So we need to use the symbol name instead of ref_reloc_sym in case of
return probes.

We only support swap file calling nfs_direct_IO. However, application
might be able to get to nfs_direct_IO if it toggles O_DIRECT flag
during IO and it can deadlock because we grab inode->i_mutex in
nfs_file_direct_write(). So return 0 for such case. Then the generic
layer will fall back to buffer IO.

This function call was being optimized out during nfs_fhget(), leading
to situations where we have a valid fileid but still want to use the
mounted_on_fileid.  For example, imagine we have our server configured
like this:

If our client mounts /exports and tries to do a "chown -R" across the
entire mountpoint, we will get a nasty message warning us about a circular
directory structure.  Running chown with strace tells me that each directory
has the same device and inode number:

With this patch the mounted_on_fileid values are used for st_ino, so the
directory loop warning isn't reported.

Without this the aux port does not get detected, and consequently the touchpad
will not work.

This patch solves deadlock between clock prepare mutex and regmap mutex reported
by Tomasz Figa in [1] by implementing solution from [2]: "always leave the clock
of the i2c controller in a prepared state".

On each i2c transfer handled by s3c24xx_i2c_xfer(), clk_prepare_enable() was
called, which calls clk_prepare() then clk_enable(). clk_prepare() takes
prepare_lock mutex before proceeding. Note that i2c transfer functions are
invoked from many places in kernel, typically with some other additional lock
held.

It may happen that function on CPU1 (e.g. regmap_update_bits()) has taken a
mutex (i.e. regmap lock mutex) then it attempts i2c communication in order to
proceed (so it needs to obtain clock related prepare_lock mutex during transfer
preparation stage due to clk_prepare() call). At the same time other task on
CPU0 wants to operate on clock (e.g. to (un)prepare clock for some other reason)
so it has taken prepare_lock mutex.

Implemented solution from [2] leaves i2c clock prepared. Preparation is done in
s3c24xx_i2c_probe() function. Without this patch, it is immediately unprepared
by clk_disable_unprepare() call. I've replaced this call with clk_disable() and
I've added clk_unprepare() call in s3c24xx_i2c_remove().

The s3c24xx_i2c_xfer() function now uses clk_enable() instead of
clk_prepare_enable() (and clk_disable() instead of clk_unprepare_disable()).

commit 6e3f62f (mfd: core: Fix platform-device id generation) modified
the computation of the mfd cell id. Negative numbers forbit the specification
of cell ids as we do. Fix this for now by specifying a base of 0 instead. In
the long run, this may be changed to automatic cell ids (base -2).

I recently posted a patch ("storage: Add quirk for another SCM-based
USB-SCSI converter") to add a quirk for the converter with ID 04E6:000F,
which is listed along with 04E6:000B in the Windows INF file for the
Startech ICUSBSCSI2 as "eUSB SCSI Adapter (Bus Powered)".

The already-present quirk for 04E6:000B has USB_SC_SCSI/USB_PR_BULK, not
USB_SC_DEVICE/USB_PR_DEVICE. Change the 04E6:000F quirk to match that,
since it will probably be required.

Commit 8eb23b9 ("sched: Debug nested sleeps") added code to report
on nested sleep conditions, which we generally want to avoid because the
inner sleeping operation can re-set the thread state to TASK_RUNNING,
but that will then cause the outer sleep loop not actually sleep when it
calls schedule.

However, that's actually valid traditional behavior, with the inner
sleep being some fairly rare case (like taking a sleeping lock that
normally doesn't actually need to sleep).

And the debug code would actually change the state of the task to
TASK_RUNNING internally, which makes that kind of traditional and
working code not work at all, because now the nested sleep doesn't just
sometimes cause the outer one to not block, but will cause it to happen
every time.

In particular, it will cause the cardbus kernel daemon (pccardd) to
basically busy-loop doing scheduling, converting a laptop into a heater,
as reported by Bruno Prémont.  But there may be other legacy uses of
that nested sleep model in other drivers that are also likely to never
get converted to the new model.

This fixes both cases:

 - don't set TASK_RUNNING when the nested condition happens (note: even
   if WARN_ONCE() only _warns_ once, the return value isn't whether the
   warning happened, but whether the condition for the warning was true.
   So despite the warning only happening once, the "if (WARN_ON(..))"
   would trigger for every nested sleep.

 - in the cases where we knowingly disable the warning by using
   "sched_annotate_sleep()", don't change the task state (that is used
   for all core scheduling decisions), instead use '->task_state_change'
   that is used for the debugging decision itself.

(Credit for the second part of the fix goes to Oleg Nesterov: "Can't we
avoid this subtle change in behaviour DEBUG_ATOMIC_SLEEP adds?" with the
suggested change to use 'task_state_change' as part of the test)

Dave Chinner has properly pointed out that this is a deadlock in the
reclaim code because ext4 doesn't submit pages which are marked by
PG_writeback right away.

The heuristic was introduced by commit e62e384 ("memcg: prevent OOM
with too many dirty pages") and it was applied only when may_enter_fs
was specified.  The code has been changed by c3b94f4 ("memcg:
further prevent OOM with too many dirty pages") which has removed the
__GFP_FS restriction with a reasoning that we do not get into the fs
code.  But this is not sufficient apparently because the fs doesn't
necessarily submit pages marked PG_writeback for IO right away.

ext4_bio_write_page calls io_submit_add_bh but that doesn't necessarily
submit the bio.  Instead it tries to map more pages into the bio and
mpage_map_one_extent might trigger memcg charge which might end up
waiting on a page which is marked PG_writeback but hasn't been submitted
yet so we would end up waiting for something that never finishes.

Fix this issue by replacing __GFP_IO by may_enter_fs check (for case 2)
before we go to wait on the writeback.  The page fault path, which is
the only path that triggers memcg oom killer since 3.12, shouldn't
require GFP_NOFS and so we shouldn't reintroduce the premature OOM
killer issue which was originally addressed by the heuristic.

As per David Chinner the xfs is doing similar thing since 2.6.15 already
so ext4 is not the only affected filesystem.  Moreover he notes:

: For example: IO completion might require unwritten extent conversion
: which executes filesystem transactions and GFP_NOFS allocations. The
: writeback flag on the pages can not be cleared until unwritten
: extent conversion completes. Hence memory reclaim cannot wait on
: page writeback to complete in GFP_NOFS context because it is not
: safe to do so, memcg reclaim or otherwise.

Currently exception occures due to access beyond buffer_iter
range while using index of cpu bigger than num_possible_cpus().
Below there is an example for such exception when we use
cpus 0,1,16,17.

In order to fix buffer allocation size for non-continuous cpu ids
we allocate according to the max cpu id and not according to the
amount of possible cpus.

Consider the following v2 hierarchy.

  P0 (+memory) --- P1 (-memory) --- A

P0 has memory enabled in its subtree_control while P1 doesn't.  If
both A and B contain processes, they would belong to the memory css of
P1.  Now if memory is enabled on P1's subtree_control, memory csses
should be created on both A and B and A's processes should be moved to
the former and B's processes the latter.  IOW, enabling controllers
can cause atomic migrations into different csses.

The core cgroup migration logic has been updated accordingly but the
controller migration methods haven't and still assume that all tasks
migrate to a single target css; furthermore, the methods were fed the
css in which subtree_control was updated which is the parent of the
target csses.  pids controller depends on the migration methods to
move charges and this made the controller attribute charges to the
wrong csses often triggering the following warning by driving a
counter negative.

This patch fixes the bug by removing @css parameter from the three
migration methods, ->can_attach, ->cancel_attach() and ->attach() and
updating cgroup_taskset iteration helpers also return the destination
css in addition to the task being migrated.  All controllers are
updated accordingly.

* Controllers which don't care whether there are one or multiple
  target csses can be converted trivially.  cpu, io, freezer, perf,
  netclassid and netprio fall in this category.

* cpuset's current implementation assumes that there's single source
  and destination and thus doesn't support v2 hierarchy already.  The
  only change made by this patchset is how that single destination css
  is obtained.

* memory migration path already doesn't do anything on v2.  How the
  single destination css is obtained is updated and the prep stage of
  mem_cgroup_can_attach() is reordered to accomodate the change.

* pids is the only controller which was affected by this bug.  It now
  correctly handles multi-destination migrations and no longer causes
  counter underflow from incorrect accounting.

Dmitry reported a lockdep splat [1] (false positive) that we can fix

ALSA sequencer core has a mechanism to load the enumerated devices
automatically, and it's performed in an off-load work.  This seems
causing some race when a sequencer is removed while the pending

Patch series "optimize memory hotplug", v3.

This patchset:

 - Improves hotplug performance by eliminating a number of struct page
   traverses during memory hotplug.

 - Fixes some issues with hotplugging, where boundaries were not
   properly checked. And on x86 block size was not properly aligned with
   end of memory

 - Also, potentially improves boot performance by eliminating condition
   from __init_single_page().

 - Adds robustness by verifying that that struct pages are correctly
   poisoned when flags are accessed.

The following experiments were performed on Xeon(R) CPU E7-8895 v3 @
2.60GHz with 1T RAM:

booting in qemu with 960G of memory, time to initialize struct pages:

This patch (of 6):

Start qemu with the following arguments:

  -m 64G,slots=2,maxmem=66G -object memory-backend-ram,id=mem1,size=2G

Which: boots machine with 64G, and adds a device mem1 with 2G which can
be hotplugged later.

Also make sure that config has the following turned on:
  CONFIG_MEMORY_HOTPLUG
  CONFIG_MEMORY_HOTPLUG_DEFAULT_ONLINE
  CONFIG_ACPI_HOTPLUG_MEMORY

Using the qemu monitor hotplug the memory (make sure config has (qemu)

The problem is detected in: drivers/base/memory.c

   static bool pages_correctly_reserved(unsigned long start_pfn)

This function loops through every section in the newly added memory
block and verifies that the first pfn is valid, meaning section exists,
has mapping (struct page array), and is online.

The block size on x86 is usually 128M, but when machine is booted with
more than 64G of memory, the block size is changed to 2G: $ cat
/sys/devices/system/memory/block_size_bytes 80000000

or

   $ dmesg | grep "block size"

During memory hotplug, and hotremove we verify that the range is section
size aligned, but we actually must verify that it is block size aligned,
because that is the proper unit for hotplug operations.  See:
Documentation/memory-hotplug.txt

So, when the start_pfn of newly added memory is not block size aligned,
we can get a memory block that has only part of it with properly
populated sections.

In our case the start_pfn starts from the last_pfn (end of physical
memory).

   $ dmesg | grep last_pfn
   [    0.000000] e820: last_pfn = 0x1040000 max_arch_pfn = 0x400000000

0x1040000 == 65G, and so is not 2G aligned!

The fix is to enforce that memory that is hotplugged and hotremoved is
block size aligned.

With this fix, running the above sequence yield to the following result:

syzkaller reproducer runs a bunch of threads that constantly switch
between DEADLINE and NORMAL classes while interacting through futexes.

The splat above is caused by the fact that if a DEADLINE task is setattr
back to NORMAL while in non_contending state (blocked on a futex -
inactive timer armed), its contribution to running_bw is not removed
before sub_rq_bw() gets called (!task_on_rq_queued() branch) and the
latter sees running_bw > this_bw.

Fix it by removing a task contribution from running_bw if the task is
not queued and in non_contending state while switched to a different
class.

In validate_checkpoint(), if we failed to call get_checkpoint_version(), we
will pass returned invalid page pointer into f2fs_put_page, cause accessing
invalid memory, this patch tries to handle error path correctly to fix this
issue.

Increase kasan instrumented kernel stack size from 32k to 64k. Other
architectures seems to get away with just doubling kernel stack size under
kasan, but on s390 this appears to be not enough due to bigger frame size.
The particular pain point is kasan inlined checks (CONFIG_KASAN_INLINE

In case devlink reload failed, it is possible to trigger a
use-after-free when querying the kernel for device info via 'devlink dev
info' [1].

This happens because as part of the reload error path the PCI command
interface is de-initialized and its mailboxes are freed. When the
devlink '->info_get()' callback is invoked the device is queried via the
command interface and the freed mailboxes are accessed.

Fix this by initializing the command interface once during probe and not
during every reload.

This is consistent with the other bus used by mlxsw (i.e., 'mlxsw_i2c')
and also allows user space to query the running firmware version (for
example) from the device after a failed reload.

Fix this by using dev_name(handle->dev) which guarantees that the name is

The trace_hardirqs_{on,off}() require the caller to setup frame pointer
properly. This because these two functions use macro 'CALLER_ADDR1' (aka.
__builtin_return_address(1)) to acquire caller info. If the $fp is used
for other purpose, the code generated this macro (as below) could trigger
memory access fault.

This because the $fp(aka. $s0) register is not used as frame pointer in the
assembly entry code.
